These are legal documents for the school. We must have the original signed forms on file. Please print them out and complete them. Mail or return the original signed form to the Middle School Nurse's Office. Please do not return these forms electronically.
This document must be signed by a parent and each student and kept on file in the building. It outlines appropriate use of the District's computer network, the Internet, and District owned hardware.
